Care management and advocacy group working with disabled and senior clients is looking for Client Care Manager analyze a client’s situation, research and create strategies for a Plan of Care.

Hours/Schedule: 10-20 hours/week but will flux up and down as need arises.

Job Type: Contract to Hire

Location Requirements: Hybrid with some work required at the client’s site. Flexible schedule so you can arrange for the days on site and remote.

Rate: $30/hour

Job Description: Client Care Manager (CCM) serves elderly and disabled clients.  The CCM is a trusted advisor who will analyze a client’s situation and then research and create strategies within a Plan of Care that assist and contribute to a well-rounded quality of life These components include housing, finances, emotional stability, and living conditions.  The CCM serves as a liaison between the client and various service providers (i.e. in-home care companies, movers, organizers, utility companies, etc.).  The CCM works collaboratively with the RN Care Manager to ensure that resources are coordinated between medical and non-medical needs for each client.  Also, the Client Care Manager collaborates with the Daily Money Manager on behalf of their mutual client.   This is NOT home health care or skilled nursing services.

Qualifications:

  • BA or BS degree
  • Minimum of two years of management experience—project management experience preferred
  • Self-motivated and able to multi-task and manage multiple priorities
  • Proficient using a computer, smart phone, and Microsoft Office products
  • Must have a valid driver’s license, current car insurance, and a reliable mode of transportation
  • Must pass civil and criminal background checks
  • Ability to work independently and as a team player
  • Strong attention to detail and ability to shift priorities quickly
  • Comfortable working virtually and in person
  • Excellent written and verbal communication skills
  • Compassionate with a sense of humor
  • Must be able to relate to small business owners
